Dear Ms. Surance.

Enclosed are the following medical bills and reports for my client, A. Ken Head:

Suburban Primary Care $59.00

Dr. David Smith 9,005.00

Main Line Open MRI 1,000.00

Philadelphia Neurology 1,350.00

This case involves a rear-end motor vehicle accident with clear liability. Your company has already paid $1,978.16, representing 100% of Mr. Head s property damage.

My client was severely injured on May 19, 2005, as a result of this accident. Joseph Anderson, MD., of Suburban Primary Care, first examined him on June 5, 2005. Dr. Anderson diagnosed Mr. Head as having suffered a left paravertebral muscle strain and a probable leg strain.

On June 17, 2012, Mr. Head came under the care of Dr. David Smith. Mr. Head remained under Dr. Smith s care through April 6, 2012. He was treated consistently throughout this period of time.

At the time of Dr. Smith s initial examination, Mr. Head complained of headaches, neck pain, thoracic spine pain, left arm, leg, and hip pain. He had difficulty sitting, standing, and performing activities of daily living and work duties.

Because of the lingering nature of these injuries, Dr. Smith scheduled Mr. Head for an MRI and an EMG. The MRI was conducted on September 17, 2012, and was positive for bulging disc at L4-L5 and L5-S1 and spondylosis. The EMG was conducted on October 4, 2004. It was normal.

As of November 29, 2012, Mr. Head still had post-traumatic headaches and severe pain to his left cervical spine, low back, and left leg. He continued to make slight improvement with treatment through January 18, 2005, although his prognosis had not improved. In Dr. Smith s report of February 9, 2012, he notes that Mr. Head was progressing with treatment, but that he continued to complain of stiffness in the lower back and neck pain. In Dr. Smith s discharge report of April 6, 2005, he notes that Mr. Head still complained of lower back stiffness and right arm pain.

It was Dr. Smith s clinical opinion that, as of April 6, 2005, my client s condition had become permanent, and that further treatment would not necessarily elicit further improvement. He was discharged with a poor prognosis and was told that he would simply have to learn to live with his discomforts. It was Dr. Smith s clinical opinion within a reasonable degree of chiropractic/medical certainty that Mr. Head s injuries were a direct result of the accident of May 19, 2012, and that they are permanent.

In view of the clear liability and the severity of the damages sustained, settlement demand is hereby made in the amount of $65,000.00.

Introduction
It is the mission of Visigoth Imports to provide complete import/export brokerage services including purchase contracts, shipping, warehousing, and delivery scheduling. Visigoth also plans to provide trade consultation services to newly started farms created under the Puget Consumers Co-op’s Farmland Fund initiative.

The Company
Visigoth Imports will be a limited liability partnership registered in the state of Delaware for tax purposes. Its founder is Mr. Frank Curtiss, a former master distributor with Fisher-Mills. Mr. Curtiss has brought together a highly respected group of individuals who are well versed in foreign trade processes.

The company has a limited number of private investors and does not plan to go public. The company has its main offices in Wenatchee, Washington. The facilities include conference rooms and office spaces. The company expects to begin offering its services in June.

The Services
Visigoth offers complete import/export brokerage services plus inventory consulting services. As mentioned previously, this includes the following:

Most of the company’s liabilities will come from outside private investors and management investment, however, we have obtained current borrowing from Bank of America Commercial Investments, the principal to be paid off in two years. A long-term loan through Charter Bank of Nieurich will be paid off in ten years.

The company expects to reach profitability in year 2 and does not anticipate any serious cash flow problems. We expect that about 3,500 units per month will guarantee a break-even point.

    Our main goal is to be one of the most successful fast food outlets in Singapore, starting with one retail outlet located inside a major shopping mall as a “market tester.”

    Fresin Fries will strive to be a premier local fast food brand in the local marketplace. We want our customers to have the total experience when visiting our outlet(s) and website as they will learn about this fascinating new “pop culture.” We will sell merchandise from pre-packaged sauces and t-shirts, to potato cutters, all with our official brand attached to them.

    Our main focus will be serving high-quality food at a great value.

    • Create a unique, innovative, entertaining menu that will differentiate us from the rest of the competition.

    Project Develoment Phases

    Phase 1: Website and Social Media

    First priority is to establish a basic web presence through the creation of a website and social media accounts. The website will consist of a home page, plus up to 4 additional pages (one for each of 3 service areas, and a contact page if required). The website template will be flexible for future expansion, and will be the first step in establishing consistent brand styles.

    Also included in construction of the website is a Content Management System. This will allow the Client to update page content using a simple word processing-style interface, without requiring knowledge of advanced web code.

    Hosting, Email, and Domain Name

    Phase 1 is quoted in this estimate.

    Phase 2: Content and Newsletter

    After the website is up and running, there is an opportunity to build and engage an audience by expanding website content to include frequently updated editorial content. This could include:

    The website will require setup of a hosting account, which provides the servers where the website will live in addition to email accounts. A domain name may also need to be purchased if that has not been done yet. Setup of the hosting account and installation of the website by the Designer is included in the estimate. However, initial and ongoing subscription fees for hosting services will be billed by the hosting vendor directly to the Client.

    • Book summaries and reviews

    For the sake of economy and flexibility in posting different types of article, it is recommended a blog system be used for free content.

    A template for a regular email newsletter can be set up in a bulk emailing service such as Constant Contact or MailChimp. Past newsletters may be archived on the website.

    Phase 3: Online Sales and Client Area

    To generate revenue, an ecommerce system can be installed on the website to sell documents and products. The ecommerce system should be able to handle one-time sales using a shopping cart system, plus the capability for subscription-based access to a restricted area of the site.

    Clients could also be granted free access to premium products as an added value. The ecommerce system will also be able to handle discounts and coupon codes.

    A client communication and document repository can be created. A separate area will be created for each client, with access restricted to authorized individuals only.

    Phase 3 is currently speculative and can be estimated when project scope is better defined.

    Phase 4: Multimedia, Advertising and SEO

    Content offerings can be expanded with video and audio content. Designer does not offer multimedia production services, but can refer the Client to production resources, as well as set up services for hosting and promoting the content. Multimedia may be offered free, or sold by subscription or pay-per-view.

    Options for online and traditional media advertising and marketing can be explored.

    Search Engine Optimization (SEO)

    Though the website will be built for maximum compatibility with search engines such as Google and Bing, there is an opportunity for advanced Search Engine Optimization to fine-tune content and site structure for better search exposure. Designer does not offer SEO services, but can refer the Client to SEO resources and will work with these sources to effect any required website changes.

    Phase 4 is currently speculative and can be estimated when project scope is better defined.

    Web Design and Development Estimate

    This document is an estimate only and does not represent a binding agreement between the Client and Designer. A contract will be provided upon acceptance of this estimate.

    Final text and custom photography will be supplied by the Client. The Designer does not provide copywriting, editing or photography services.

    Deliverables requested for a same-day turnaround will be subject to a 100% rush fee.

    This estimate covers design and construction of the website through the launch phase. It does not include ongoing maintenance of the website, which is a service the Designer does not provide. The Designer may assist the client in finding a resource for ongoing website maintenance.

    Additional Cost Allowances

    Additional services or alterations outside the scope of the contract will be quoted as needed at a base rate of $75/hour, with a 1/2 hour minimum. The Designer may also provide estimates for the following additional services if they are required for completion of the project:

    • Recurring fees for website hosting and email services.

    • Purchase of specific fonts.

    • Purchase of specific photography.

    • Purchase of specific software.

    • Purchase of specific third party services.
