  • Use a different browser (Google Chrome, Mozilla Firefox, Internet Explorer).

Searching on ASIC Connect

When performing a search on ASIC Connect you will need to select a tick box stating ‘I am not a robot’ to return the results.

If you are using an automated service, you may experience problems searching ASIC Connect.

If you require large amounts of data, you should consider using ASIC’s registry data on Data.gov.au.

What is an ASIC key?

Your ASIC key is a unique number used in ASIC Connect that helps us establish your identity and protects your business information by making sure only you and those you have authorised can access it.

What is an AUSkey?

AUSkey is a single key to access government online services. In the future, you may no longer need different user IDs and passwords as AUSkey becomes accepted by more government agencies for their online services.

What is a corporate key?

A corporate key is an 8-digit number uniquely associated with a company’s ACN. Your company needs only one corporate key.

Welcome to The Frugal Entrepreneur s collection of free business forms, letters, and templates for small and home-based businesses. While there are currently numerous sites online offering thousands of printable business documents and templates that are free to download, I have found that most of these offerings are poor in quality or are unsuitable to the needs of either small and home-based companies or the self-employed.

I therefore decided to put together this collection of essential business documents. I hand picked and adapted all of these forms, templates, and sample business letters to specifically suit the needs of small and home businesses. Most of these documents are from my growing personal collection of files that I have used over the years for my own business transactions. (Where any document is from an outside source, the source and link are provided.)

If there are any small business templates, forms, or letters that are not listed here, but you cannot find a suitable example or free, printable download of it anywhere, then let me know, either by leaving a comment in the box below or by sending an email (you can click the email icon on the upper right-hand side of this page), and I ll see what I can do.

And a final note You may notice that I have conspicuously left out most legal-type business documentation. When it comes to these kinds of forms, which as mentioned above, are freely available on several websites, my advice is: do yourself a favor and hire a qualified attorney or CPA to make sure you don t make a regrettable mistake down the road.

Address of record

Section 54.1-2400.02.B of the Code of Virginia requires the Department of Health Professions (DHP) to collect an official address of record from each health professional to be used by DHP for agency purposes. The Code also requires DHP to provide an opportunity for a health professional to provide a second address for the purpose of public dissemination. If there is no second (public) address provided, the official address of record will be used as the public address for the purpose of public dissemination. This is applicable to all pharmacists, pharmacy interns, and pharmacy technicians. If the individual would prefer that his/her address of record remain confidential, then an alternative public address must be provided to DHP. (Note: the License Lookup feature on the DHP public website does not contain the full address. However, public addresses are available by contacting Virginia Interactive Premium Service or the Board.) An individual is not required to submit a place of residence for either the official address of record or the public address. A post office box or a practice location is acceptable. This is not true for facilities licensed by the Board of Pharmacy. The address of record for facilities must be the actual physical location of the facility.

Changes to either address may be made at the time of renewal or at any time by written notification to the Board. Changes to the address of record may also be made at any time by accessing your licensure information through the online system. Any change that is made to one of the addresses does not automatically update the other address. Address information should be kept current for the address of record and the public address (if different from the address of record) at all times.

Facilities licensed by the Board may only make changes to their address by filing an application with the Board prior to any change, and such change typically requires an inspection and approval of the new location.

Dissolution of Marriage Procedure

We realize that filing a Dissolution of Marriage (Divorce) can be a difficult decision. We are not attorneys being paid by the hour; we charge a flat rate affordable fee. You will speak to a friendly, caring and knowledgeable case manager who will assist you with your needs. As we are not attorneys, if you require legal advice or need to know your rights, we suggest you consult with an attorney prior to starting a case with us.

STEP 1 On your initial visit or scheduled phone conference, you will provide us with the information we need to type your Divorce papers. Our fee will be paid at that time. ($250.00 without children or property) and ($450.00 with a child) and the cost of a mandatory parenting class which can be taken online costing approximately $20.00 each. Next we will schedule a follow-up appointment 5 to 10 days later depending on the complexity of your case.

STEP 2 At the second appointment, you will review the accuracy of the forms and we will make necessary changes. Your signature will then be required and we will notarize your documents free of charge. You may bring your spouse in to sign the papers at the same time or they may make an appointment to sign separately. In the event that your spouse does not agree to sign the papers in our office you do have 3 other options: 1) You can mail the papers to your spouse. They must sign their papers with a notary and return them to you to be filed in the court; or 2) File your papers in the Court first and then have the Clerk of Court forward your papers to the Sheriff to serve the papers on your spouse (additional fee, depending on where your spouse lives); or use a private process server to do the same; or 3) If you have honestly tried all means to locate your spouse and cannot locate them, the court requires a search and a publication in a local newspaper, a consultation with an attorney may be appropriate but is not required.

STEP 3 File your papers DIRECTLY with the Court along with the filing fee of $408.00. You may ask the court if you qualify to have the court costs waived! Please be aware that we will type all forms contained in the divorce packet that you request, however, the court, NOT US, will set your case for a final hearing to dissolve your marriage.

The 10 Key Legal Documents for Your Business

Documents play an essential role in protecting the interests of the business and business owners over the course of a company s lifetime. Here is a list of the 10 most common legal documents to help you determine what your business needs.

1. Company bylaws for corporations. Most states require corporations to keep a written record of bylaws, although you don t need to file the document with a state office. Bylaws define how the company will govern itself. Even if your company is incorporated in the handful of states that don t require bylaws, they are still a good idea as they spell out your business structure, individual roles, and governance issues. For example, bylaws can help settle a dispute on the length of a director s term or define if you need a simple majority to approve a decision.

2. Meeting minutes. Most states also require corporations to document what happens at major meetings. They keep an official account of what was done or talked about at formal meetings, including any decisions made or actions taken. They can help settle a dispute about what happened or didn t happen in a past meeting.

Your minutes should be detailed enough to serve as your corporation s institutional memory. They should include: type of meeting; time and place of meeting; detailed attendance; all actions taken (purchases, elections, etc.); as well as any votes including how everyone voted and who abstained.

3. Operating agreement for LLCs. Although not required in most states, an operating agreement is recommended for every LLC, particularly when there are multiple members involved. This document outlines an LLC s financial and functional decisions. If there is more than one member, it becomes all the more important to define how key business decisions will be made, how profits and losses will be distributed, what are the rights and obligations of members and what happens when someone wants out of the business. Once members sign the document, it becomes an official, binding contract.

4. Non-disclosure agreement. Whether you realize it or not, your business has information that should remain private, such as customer list, financial records, or ideas for a new pricing plan. An NDA is your first line of defense to protecting this information. This legal document creates a confidential relationship between your business and any contractors, employees, and other business partners who might get a behind-the-scenes look at your operations.

5. Employment agreement. This contract sets the obligations and expectations of the company and employee in order to minimize future disputes. Not every hire requires an employment agreement, but the document can be a useful if you want to dissuade certain new hires from leaving your company too soon, disclosing confidential information about your business, or going to work at a competitor. The contract should be reviewed by an experienced employment law attorney before given to an employee to sign.

6. Business plan. A business plan may not be a legal document, but it s required should you ever decide to seek financing or sell your business. Your business plan can be one page or a hundred pages, as long as it provides clarity on your business opportunity and your roadmap to get there.

7. Memorandum of understanding. An MOU falls somewhere between a formal contract and a handshake. It documents any important conversations you have with suppliers, potential partners and others involved in the business. MOUs are great ways to lay out the terms of a project or relationship in writing, but do not rely on the document to be legally binding.

8. Online terms of use. While not required by law, any business with a website should include their terms of use. These pages can limit your liability in cases where there are errors in your own content, as well as information contained in any hyperlinks from your website. Furthermore, your Terms should let visitors know what they can or can t do on your site, particularly in cases where visitors can comment on blogs or share their own content.

9. Online privacy policy. If you gather any information from your customers or website visitors (such as email addresses), you are legally required to post a privacy policy that outlines how this information will be used and not used.

10. Apostille. Businesses involved in international trade with other Hague Convention countries may need a certificate, known as an apostille, that authenticates the origin of a public document (like articles of incorporation) so they can be recognized in another country. Apostilles are only valid in countries that are members of the Hague Convention.

In most cases, you don t need to create any of these documents from scratch. You can find free templates online to serve as a starting point. While these legal documents are important part of staying compliant with your state requirements, they are more than empty formalities. By taking the time to think about the various elements on each document, you are setting the right foundation for your business.
