VistA Non-EHR Modules

People who are interested=2 0in the VistA health information system tend to focus primarily on the clinical applications and the Computerized Patient Record System (CPRS) in particular. However, there are numerous other modules within VistA that might be of interest to the user and vendor communities, e.g. Police Security, Library, Incident Reporting, Nursing Education, Engineering, Dentistry, etc. This paper attempts to bring some of these lesser known non-clinical care VistA software modules to your attention for consideration.

In 1982, the Veterans Health Administration (VHA) committed to building an electronic health care architecture called the Decentralized Hospital Computer Program (DHCP). The focus was the of software applications easily integrated into a complete hospital information system. VA selected the ANSI Standard ‘M’ as the primary language for DHCP, and began developing applications using VHA internal programmers who directly with user groups in software prototyping environments.

By 1990, VHA had upgraded computer capacity at all medical facilities and began implementing complex integrated software s olutions across the national health care delivery system. 1996, the VHA DHCP system was renamed and evolved into the Veterans Health Information Systems and Technology Architecture (VistA).

VistA is built on a client-server architecture, which ties together workstations and personal computers with graphical user interfaces at VA facilities, as well as software developed by local medical staff. VistA also includes the links that allow commercial off-the-shelf software and products to be used with existing and future technologies.

A key component of VistA is the Computerized Patient Record System (CPRS). It is a true longitudinal health record that follows the patient through all VA treatment settings, outpatient, inpatient, and long care. CPRS has been installed and used at all VA Medical Centers since 1997.

Other noteworthy applications are VistA Imaging and Bar Code Medication Administration (BCMA). VistA Imaging facilitates medical decision-making by delivering complete multimedia patient information to the clinician’s desktop in an integrated manner. Images and associated text data are available at all times anywhere in the hospital. BCMA provides a real-time point-of-car e solution to ensure that the right drug, the right dose, goes to the right patient at the right time.

In total, Vista includes over 130 administrative, clinical, and technical software modules (see Appendix or visit http://www.va.gov/VISTA_MONOGRAPH/ ).

Selected Examples of Non-EHR VistA Modules

The purpose of the Patient Representative module is to ensure that medical centers are more responsive to patient needs. The software tracks and trends compliments and complaints and measures the facility’s types of complaints as they relate to the Customer Services Standards and the National Patient Satisfaction Survey. This package supports the Patient Advocate with the collection and categorization of complaints and compliments that give the medical center an opportunity to meet and exceed the customer’s expectations.

Engineering, also known as Automated Engineering Management System/Medical Equipment Reporting System (AEMS/MERS), facilitates the management of information needed to effectively discharge key operational responsibilities normally assigned to VA engineering organizations, such as:

* Uses bar codes for equipment inventory and preventive maintenance. Completed work orders are automatically posted to equipment histories.

* Tracks and controls work orders, maintaining annotated repair histories for medical and non­-medical equipment. There is a separate menu option for display of incomplete work orders.

* Identifies building features (square footage, floor coverings, window types, etc.) and keeps track of locks and keys.

The Nursing application is a comprised of multiple modules (i.e. Administration, Education, Clinical, and Quality Assurance).

The nursing software includes the following functionalities:

· Tracks staff information.

· Accumulates daily statistics on the number of patients treated.

Generates daily, monthly, quarterly, and yearly management reports.

· Support nurse educators by providing data/information for the tracking of mandatory

· programs and staff education.

· Includes nationally developed standard nursing care plans for initiating patient care plan generation.

· Allows nurses to generate a patient care plan based on patient problems, identified goals, and specified nursing interventions.

Police and Security

The Police and Security module supports the VA Police in their responsibilities of crime prevention, preliminary investigation of crimes, apprehension, legally correct handling of suspected offenders, and the transfer of suspected offenders to appropriate authorities.

* Serves as the repository file for the names, addresses, and other demographic data of persons who come in contact with the Police and Security staff.

* Allows for the retrieval and display of stored information for a selected person.

* Stores electronic data normally entered on the VA Police Uniform Offense Report, Investigative Notes, Case Log, and other documentation assembled during an investigation.

* Contains all violation information on U.S. District Court Violations Notices and Courtesy Warnings issued by Police Officers.

* Assembles selective data into a standardized report format documenting the numbers and types of criminal incidents that are occurring at the site.

* Records information necessary for the Vehicle Registration program.

* Records any data pertinent to individuals currently under any type of criminal proceedings or other legal restraining document.

Conclusions Recommendations

VistA is a comprehensive system containing more than just clinical applications tied to the EHR. It has quite a few other administrative, management and technical software applications that can stand alone and be used for other purposes. The Engineering system could be used by almost any organization to keep track of its equipment, offices, maintenance records, and associated work orders. The Nursing Education module could be used by any Nursing school or department to track and keep records on credentials and training received. There are modules to track customer complaints, track volunteer hours, motor vehicles, library holdings, and much more. The open source software development community ought to seriously consider creating the GUI front end to these modules and tailoring them to meet the needs of many medical and non-medical organizations. Admission requirements

The department maintains different admission requirements for the campus-based and online programs, which provide separate focus areas. The concentration of the campus-based program is in Theory and Research, while the concentration for the online program is in Justice Policy and Administration. For both, you�ll need to meet the admission requirements for the graduate school. The additional requirements for each program are outlined below.

Campus-based program (Concentration in Theory and Research):

  • Completed application on file with the graduate school
  • Verbal reasoning and analytical writing GRE scores on file with the graduate school
  • Personal statement
  • Transcripts on file with the graduate school with a minimum overall 3.0 GPA on undergraduate work

Online program (Concentration in Justice Policy and Administration):

Grants for starting a business: What small business grants are available?

If you’re a small business owner or thinking of starting a business, finance is probably your biggest concern. You may be wondering if there is any funding available for start-ups.

The simple answer is yes, but getting your hands on it can be a complicated and rather stressful process. There are countless schemes, each with their own set of criteria, which you can apply for when you’re on the verge of starting a business.

Start-up funding is out there to be claimed, yet every year we receive stories of piles of cash sitting in accounts and not being invested. It’s not because you don’t need the cash, but it’s because navigating through the grants jungle can leave you wishing you were still employed.

Types of business support

All publicly funded schemes are designed to encourage new and growing businesses, to bring wealth and ultimately create jobs. To help achieve this the government makes available a portion of taxpayers’ money to help and encourage enterprise through small business grants.

This cash gets distributed through a variety of ministries, departments, agencies and quangos on a national and local basis. Most businesses are eligible at any one time to apply for a number of different business start-up grants and support schemes which are distributed in a wide variety of forms.

Government business grants available

Direct grant

This is a cash award, which is usually given out for activities such as training, employment, export development, recruitment and capital investment projects. With a direct grant most schemes usually require the company involved to put up around 50% of the cost.

The government s ‘business finance support finder’ directory has over 300 direct grants agencies listed such as UK business “innovation” organisation the Technology Strategy Board. The directory also features various local grants providers which are facilitated by the government’s £3.2bn Regional Growth Fund. such as Catalyst for Growth which has direct grants of £5,000 to £500,000 available for chemical start-ups launching in the North West of England.

Soft loan

A soft loan is a special type of grant where the terms and conditions of repayment are more generous (or softer) than they would be under normal financial circumstances. So, for example, the interest rates may be less, or there may be no interest to pay at all, and the repayment terms could also be for a longer period.

There are hundreds of organisations that offer soft loans and guarantees but the most notable is government-funded scheme Start Up Loans which offers new businesses loans of up to £25,000 for 6% interest with a 12 month repayment “holiday”. If you’re starting a social enterprise or charity there are organisations such as Big Issue Invest. the investment arm of the Big Issue, which has soft loans from £50,000 to £1m available for positive impact, socially-driven entrepreneurs and also operates ‘participation loans’ where repayment is linked to future performance of the enterprise.

For young entrepreneurs aged between 18-30, The Prince s Trust provides soft loans of up to £4,000, as well as support for your business idea, with interest capped at 3% and repayments spread over a period of two to five years.

Equity finance

With equity finance a capital sum is injected into the business and the provider of the funds takes an equity share of the enterprise. When the value of the firm increases the stake can then be returned. However, unlike venture capitalists, the expectations and requirements of the providers of public funds are usually less demanding.

It is also worth noting the government Enterprise Investment Scheme (EIS) and Seed Enterprise Investment Scheme (SEIS) which help companies raise finance by providing tax relief to investors who buy a share in your business.

Free or subsidised consultancy

Start-ups can often find themselves in the situation where they are lacking a particular set of skills and there are some specially run schemes which provide this knowledge either for free or at subsidised rates. For instance, the Welsh government runs a business support service offering free, independent advice on starting a business and operates an instant hotline for business queries.

Access to resources

As with a lack of skills, it can be the case that start-ups do not possess the physical resources or facilities they need in order to develop particular projects. In the same way there are a number of initiatives, particularly incubator and accelerator schemes, that can help overcome these concerns by providing access to publicly owned facilities. One such initiative is the department for business, innovation and skills’ (DBIS) GrowthAccelerator which provides advice and coaching to fast-growth businesses with fewer than 250 employees.

Technology and Best Practice transfer

The transfer of technological advances and new best practice initiatives can often take a long time filtering down to smaller businesses. The government has set up schemes which aim to overcome this through business support networks and there is now a number of well-established best practice initiatives such as Investors in People (IIP), which specialises in business tools and resources.

Cost sharing

When it comes to research and development, the costs involved can prevent small firms from taking part. However, by sharing the costs with other businesses, and then sharing the expertise, this problem can be avoided.

4. What kinds of government loans are available to homebuyers?

Several federal, state, and local government financing programs are available to homebuyers. The two main federal programs are:

FHA loans. The Federal Housing Administration (FHA), an agency of the Department of Housing and Urban Development (HUD), offers insurance backing to loans made to U.S. citizens, permanent residents, and noncitizens with work permits who meet financial qualification rules. Under its most popular program, if the buyer defaults and the lender forecloses, the FHA pays 100% of the amount insured. This loan insurance lets qualified people buy affordable houses. The major attraction of an FHA-insured loan is that people with passable credit scores can put down as little as 3.5%. Another plus is that, unlike with conventional loans, FHA-backed loans allow the entire down payment and costs costs to come from a gift (perhaps from family). The downside is that FHA loans have a maximum loan limit that varies depending on the average cost of housing in a given region. VA loans. U.S. Department of Veterans Affairs (VA) loans are available to men and women who are now in the military and to veterans with honorable discharges who meet specific eligibility rules relating to length of service, credit history, and recent employment, as well as eligible surviving spouses. The VA doesn’t make mortgage loans, but guarantees part of the house loan you get from a bank, savings and loan, or other private lender. If you default, the VA pays the lender the amount guaranteed and you, in turn, will owe the VA. This guarantee makes it easier for veterans to get favorable loan terms with low or no down payment and no PMI (private mortgage insurance). (Conventional mortgages have PMI and FHA loans have MIP.) Other government loan programs. For information on other government loans, contact your state and local housing offices. They often have programs available for first-time homebuyers who are purchasing modestly priced properties. Grants for starting a business: What small business grants are available?

The simple answer is yes, but getting your hands on it can be a complicated and rather stressful process. There are countless schemes, each with their own set of criteria, which you can apply for when you’re on the verge of starting a business.

Start-up funding is out there to be claimed, yet every year we receive stories of piles of cash sitting in accounts and not being invested. It’s not because you don’t need the cash, but it’s because navigating through the grants jungle can leave you wishing you were still employed.

Types of business support

All publicly funded schemes are designed to encourage new and growing businesses, to bring wealth and ultimately create jobs. To help achieve this the government makes available a portion of taxpayers’ money to help and encourage enterprise through small business grants.

This cash gets distributed through a variety of ministries, departments, agencies and quangos on a national and local basis. Most businesses are eligible at any one time to apply for a number of different business start-up grants and support schemes which are distributed in a wide variety of forms.

Government business grants available

Direct grant

This is a cash award, which is usually given out for activities such as training, employment, export development, recruitment and capital investment projects. With a direct grant most schemes usually require the company involved to put up around 50% of the cost.

Soft loan

A soft loan is a special type of grant where the terms and conditions of repayment are more generous (or softer) than they would be under normal financial circumstances. So, for example, the interest rates may be less, or there may be no interest to pay at all, and the repayment terms could also be for a longer period.

There are hundreds of organisations that offer soft loans and guarantees but the most notable is government-funded scheme Start Up Loans which offers new businesses loans of up to £25,000 for 6% interest with a 12 month repayment “holiday”. If you’re starting a social enterprise or charity there are organisations such as Big Issue Invest. the investment arm of the Big Issue, which has soft loans from £50,000 to £1m available for positive impact, socially-driven entrepreneurs and also operates ‘participation loans’ where repayment is linked to future performance of the enterprise.

For young entrepreneurs aged between 18-30, The Prince s Trust provides soft loans of up to £4,000, as well as support for your business idea, with interest capped at 3% and repayments spread over a period of two to five years.

Equity finance

With equity finance a capital sum is injected into the business and the provider of the funds takes an equity share of the enterprise. When the value of the firm increases the stake can then be returned. However, unlike venture capitalists, the expectations and requirements of the providers of public funds are usually less demanding.

It is also worth noting the government Enterprise Investment Scheme (EIS) and Seed Enterprise Investment Scheme (SEIS) which help companies raise finance by providing tax relief to investors who buy a share in your business.

Free or subsidised consultancy

Start-ups can often find themselves in the situation where they are lacking a particular set of skills and there are some specially run schemes which provide this knowledge either for free or at subsidised rates. For instance, the Welsh government runs a business support service offering free, independent advice on starting a business and operates an instant hotline for business queries.

Access to resources

As with a lack of skills, it can be the case that start-ups do not possess the physical resources or facilities they need in order to develop particular projects. In the same way there are a number of initiatives, particularly incubator and accelerator schemes, that can help overcome these concerns by providing access to publicly owned facilities. One such initiative is the department for business, innovation and skills’ (DBIS) GrowthAccelerator which provides advice and coaching to fast-growth businesses with fewer than 250 employees.

Technology and Best Practice transfer

The transfer of technological advances and new best practice initiatives can often take a long time filtering down to smaller businesses. The government has set up schemes which aim to overcome this through business support networks and there is now a number of well-established best practice initiatives such as Investors in People (IIP), which specialises in business tools and resources.

Cost sharing

When it comes to research and development, the costs involved can prevent small firms from taking part. However, by sharing the costs with other businesses, and then sharing the expertise, this problem can be avoided.
